I was very surprised the death rate in the U.S. was twice as a high as the 70's--I had no idea it had climbed that much. I dunno about non-compliance, since that's not really a new phenomenon. My best guess is the hike comes from a combination of increased industrialization and air qualilty standards that have never matched up. Purely a guess, though.
